Coordination of Services
Our team of caring dedicated clinicians at Meadville Medical Center (MMC) will work with your physicians, healthcare providers and other agencies to help you manage chronic disease conditions, with a focus on meeting your health and wellness goals. Our team includes a medical director, physicians, registered nurses, dietitians, social workers, counselors and health coaches. Services are provided at no charge.

Children’s Health Network
We have a multi-disciplinary team of clinicians available—including a medical director, registered nurses, dieticians, counselors, social workers, and physicians—who can work with healthcare providers, physicians, and other agencies to reach your child’s health and wellness goals and needs. These services are provided without charge.
