Children's Health Network
The Children's Health Network (CHN) is the first project of its kind
in our area designed to affect the long-term health of the children in
our community. The CHN aims to improve the long-term health of children
in our area that are stricken with chronic illnesses by working one-on-one
with the patient and their family. In addition, the program educates families
on healthcare prevention, which allows children to spend more time being
healthy and spend less time in the hospital. CHN will ensure a rational
continuum of care. It is also designed to create an appropriate physician
interface and support system for the family to guarantee their child will
receive enhanced care.
