blood pressure or diabetes

- Minor office surgery, including warts,
cysts, and other skin problems
- Treatment of illness and minor injuries
- Laboratory tests and blood drawing
- Routine physical examinations for
employment, sports, schools, and preventive
health care
- Pap smears and pelvic exams
- Birth control
- Newborn care
- Hearing and vision screening
- EKG
- Routine foot care
- X-ray, limited scope
- Dietary counseling
- EPSDT-Early Periodic Screening,
Diagnosis and Treatment services
Appointments
Office hours are scheduled by appointment
only, and a sincere attempt is made to
adhere to the schedule as much as possible.
Please plan to arrive for your appointment
on time, or a few minutes early, if
possible. Routine appointments should be
made at least one week in advance; however,
if you have a problem that cannot wait,
please call as early as possible so that you
can be scheduled into the same day's
schedule.
Each patient is primarily under the care of
one provider. However, because of the nature
of our practice it may occasionally be
necessary for you to be seen by one of the
other care providers. Although each care
provider has their own patients, we are a
team and one of us is available at all
times. Routine medical services are provided
by Licensed Physicians and Physician
Extenders.

The center is open five days a week. The
office telephone number is 814-373-2276 and
is answered beginning at 8:30 a.m.
Canceling Appointments
If you find it necessary to cancel your
appointment, please do so 24 hours in
advance so that we may give your time to
someone else. If the doctor must cancel
office hours, you will be notified by
telephone or by mail if time permits. It is
important that we always have your current
address and telephone number.
Fees and Insurance
Payment is expected at the time of service
for office appointments. You can help keep
costs down by paying for your appointment or
your appropriate co-payment at the time of
service. The financial responsibility for
all services rendered to the patient rests
with that patient or his family, regardless
of any insurance coverage. If you are not
able to pay at the time of service or have
questions about your bill, please discuss
this with the office manager. We do
participate in Medicare, Medicaid and Blue
Shield.
